Consent Forms

Set up and customize consent forms in Sheet Setup.  View patient consent forms in the Chart module, Progress Notes and Account module.  Consent forms support Digital Signatures. 

To create a consent form for a patient, in the Chart module toolbar, click Consent, or click the dropdown to select the form. It will preview in the Fill Sheet window.  Below is an example of the internal extraction consent form Sheet included with Open Dental.
