Referral Slips

Set up and customize referral slips in Sheet Setup.  To assign a specific referral slip sheet to a referral source, see Referral Setup.  View a patient's referral slips in their Chart module, Progress Notes and Account module.  Referral slips support Digital Signatures. 

To create a referral slip, on the patient's Referrals window, click Referral Slip, then Fill it Out.  Below is an example of the internal referral slip Sheet included with EZ2000 Plus Dental.
